They involve<\/p>\n\n\n\n<ul class=\"text-justify wp-block-list\"><li>Bandaging and compression garments<\/li><li>Lymphatic massages (drainages)<\/li><\/ul>\n\n\n\n<p class=\"text-justify\">Manual drainages, compression garnements and bandages must be done as soon as possible to try to reverse the situation, or, at least, avoid a continuous degradation.<\/p>\n\n\n\n<p class=\"text-justify\">If the lymphatic MRI shows remaining drainage (like on this image) physiotherapy can give excellent results.<\/p>\n\n\n\n<p class=\"text-justify\">These measures <strong>require specialist care<\/strong>,&#xA0;a long term commitment from the patient and are therefore expensive. They are also <strong>short-lived<\/strong> as the reduction in size of the limb is provided only as long as the compression and massaging are done regularly. Another limitation of conservative measures is that they don&#x2019;t change the limb girth as <strong>they can&#x2019;t prevent the fat deposits and fibrosis<\/strong> in the tissues as the disease progresses.<\/p>\n\n\n\n<p class=\"text-justify\">Surgical options have therefore an important role and a combination of therapies may achieve the best results.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Surgical treatment<\/h3>\n\n\n\n<p class=\"text-justify\">When the lymphatic MRI shows a&#xA0;total lack of drainage&#xA0;due to adenomectomy and radiotherapy, it is clear that a surgical treatment is indicated.<\/p>\n\n\n\n<p class=\"text-justify\">Surgical treatment falls into 2 categories:<\/p>\n\n\n\n<ul class=\"text-justify wp-block-list\"><li><b>Physiologic surgery<\/b> aims at restoring the lymphatic function.\n<ul>\n<li><b>Autologous lymphnode transplantation (<a title=\"Glossary: ALNT\" href=\"\/?p=238\">ALNT<\/a><\/b>), also called<b> microsurgical vascularized lymph node transfer (VLNT) <\/b>is a procedure where a lymphedematous recipient site is prepared by releasing scar tissue, then a <a class=\"glossaryLink\"  title=\"Glossary: Flap\"  aria-describedby=\"tt\"  data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;Flap&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Flap surgery is a technique in plastic and reconstructive surgery where any type of tissue is lifted from a donor site and moved to a recipient site with its own blood vessels.&lt;\/div&gt;\"  href=\"https:\/\/lymphoedemacenter.com\/?glossary=flap\"  data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]'  tabindex='0' role='link'>flap<\/a> containing healthy lymph nodes with an artery and vein is harvested from a donor site and placed at the recipient site. The artery and vein of the flap are then connected to an artery and vein at the recipient site.<\/li>\n<li><b>Lympholymphatic graft<\/b> is a procedure that connects a blocked lymphatic vessel to an unobstructed one using a vein or a lymphatic vessel as a graft.<\/li>\n<li><b>Lymphovenous anastomosis (LVA)<\/b> connects an obstructed lymphatic vessel to a vein. It drains the lymph fluid into the vein system and seems to be an efficient procedure in early stage of lymphedema. Usually multiple LVAs are performed simultaneously. They require supermicrosurgery done under microscope<\/li>\n<\/ul>\n<\/li><li><b>Debulking<\/b> procedures seek to reduce the volume of the affected extremity either by liposuction or by excising excessive skin and subcutaneous tissues.<\/li><\/ul>\n\n\n\n<p class=\"text-justify\"><a title=\"Discussing your options\" href=\"\/?p=68\">Discussing your treatment options<\/a>&#xA0; is done after a thorough clinical examination and lymphatic MRI.<\/p>\n\n\n\n<p class=\"text-justify\">Learn more about <a title=\"Arm lymphedema\" href=\"\/?page_id=122\">arm lymphedema surgery<\/a>&#xA0;and&#xA0;<a title=\"Leg lymphedema\" href=\"\/?page_id=124\">leg lymphedema surgery <\/a><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><a name=\"alnt-indications\"><\/a>Indications and advantages of ALNT<\/h3>\n\n\n\n<p class=\"text-justify\">Autologous lymph node transplant <a title=\"Glossary: ALNT\" href=\"\/?p=238\">ALNT<\/a>&#xA0;(or microsurgical vascularized lymph node transfer &#x2013; <strong>VLNT<\/strong>) is a surgical treatment option for lymphedema.<\/p>\n\n\n\n<ul class=\"text-justify wp-block-list\"><li><a title=\"Glossary: ALNT\" href=\"\/?p=238\">ALNT<\/a> can cure &#x2013; It brings functional lymph nodes in the affected area and restores the lymphatic function.
